I got the same thing a month or so ago and mentioned in the list.  I was
foolish enough to fill it out and then had second thoughts.  Someone is
trying to steal your password.  I got a similar thing from paypal asking
for information, and I haven't used them for  a couple of  years.  If
you did fill it out, go into eBay and change  your password.  It should
probably be reported to them also.
